Worker seriously injured after crate falls from forklift truck

HSE reported enforcement action after a crate fell from a forklift truck and seriously injured a worker. Load stability when moving crates and palletised units is a recurring cause of serious injury; the circumstances, charges and penalty are set out in HSE's own release.
